Dr. Davis is a participating provider for Anthem (Empire) Blue Cross / Blue Shield (Dental Blue 300, Grid Plus, Dental Complete), and CIGNA (DPPO).
If you have out-of-network DPPO coverage with other carriers such as Delta Dental, Guardian, MetLife, Principal, United Concordia, all other Aetna, Anthem — Empire BC/BS, and CIGNA plans; claims will be submitted electronically on your behalf to maximize the benefits to which you are entitled.
